Universal heal care in Hawaii

According to CNN/AP:
Hawaii is dropping the only state universal child health care program in the country just seven months after it launched.

Gov. Linda Lingle's administration cited budget shortfalls and other available health care options for eliminating funding for the program. A state official said families were dropping private coverage so their children would be eligible for the subsidized plan.
Barack, are you learning anything from your home state?

As far as what happened to the insurance program in Hawaii, that was a result of bad management and not paying attention to what was going on. If you have ever been covered by more than one insurance company at the same time (like if you and your spouse both have family coverage through your employers), you've probably had one of the companies come back to you telling you that you had to use the other insurance first, then this other company might cover the leftover cost.
